Based on visual observation the facility failed to provide 3 of 5 exits that are accessible. This would allow for the exits to not be used in the case of an emergency and potentially affecting 16 of the 16 residents of the facility.

Findings:
1) - During facility tour on 12/17/2010 between the hours of 8:30am and 11:30am, the exit doors at the end of two of the three halls on the 2nd floor have a special locking mag lock device with a badge swipe device., the doors are not in direct view of a nurses station due to the cross corridor doors between the nurses station and the exit doors did not have a key override device. The doors at the end of the south and west halls.

2) - During facility tour on 12/17/2010 between the hours of 8:30am and 11:30am, one of the two elevators on the 2nd floor by conference room is not in direct view of a nurses station and is not equipped with a key override device.

Based on visual observation the facility failed to provide certified inspections to the building heating equipment affecting three (3) of three (3) of the corridors in facility and potentially affecting 16 of the 16 residents of the facility.


Findings:
During facility tour on 12/17/2010 between the hours of 8:30am and 11:30am, two new boilers were installed to supply heat to the facilities patient room and areas . The boilers were installed but a inspection to certify them has not been conducted by a certified company.

Based on Interview and Documentation review, the facility failed to ensure that portable space heating devices were not located in patient care areas of the facility. This would affect 3 of 3 smoke compartments and potentially affect 16 of the 16 residents of the facility.

Findings:

1) - On December 17, 2010 at 9:00am facility Administrator stated that the facilities last boiler went down in mid September. On November 4, 2010 space heaters were ordered and placed in Seaside patient rooms. On December 13, 2010 a team of DHH surveyors found space heaters in the patient rooms and ordered a (IJ) Immediate jeopardy condition for reasons with facility not having heat and patient risk from the cords on the space heaters due the type of patients within the facility. On December 13, 2010 facility increased its staff for constant visual of the space heaters because DHH surveyors had concerns on the 5 minute checks. On December 15, 2010 the (IJ) Immediate jeopardy was removed and lifted because thicker blankets were purchased. On December 15, 2010 the facility had one of the two replaced boilers back in service.

2) - On December 17, 2010 at 9:00am review of emails provided by the facility Administrator stated that space heaters were ordered for Seaside rooms due to a cold snap, and following up with Johnson Control for timeline on the two new boilers.

Based on visual observation the facility failed to maintain smoke dampers in HVAC ducts in smoke barriers through-out facility.. The deficient practice had the potential to affect 1of 3 smoke compartments, and approximately 16 of 16 residents of the facility..

Findings:

During facility tour on 12/17/2010, between the hours of 8:30am and 11:30am, it was observed that the smoke/fire dampers located within the HVAC duct above the ceiling on the 2nd floor by room #209, that penetrated the smoke barrier, did not have a access panel so that you can check or work on in the event of break down are regular maintenance.
