Based on on observation and interview with staff, the facility failed to provide documentation of testing and maintenance of the fire alarm system to include all equipment connected to the system in accordance with NFPA 70 and NFPA 72. 9.6.1.4 Findings,
The facility failed to provide a complete list of equipment connected to the fire alarm system to the vendor doing the testing so that all equipment will be tested. The combination fire and smoke dampers were not included on the annual test.

The damper in the powered exhaust duct penetrating the 2-hour wall on the west side of the unit was found to be in the closed position and thus preventing the exhaust system for the bathroom vents, soiled holding rooms, and others from being able to function.
