Based on record reviews and interviews, the hospital failed to provide an individual responsible for Outpatient Services, in that, no one individual was responsible for all three Outpatient Services offered by the hospital (Surgical Services, Laboratory Services, and Imaging Services).

Findings included:

1) The organizational chart did not list Outpatient Services.
2) On 8/4/15 at 1:15 PM, Staff #1 was asked who was responsible for Outpatient Services. Staff #1 stated the three areas that provided Outpatient Services (Surgical Services, Laboratory Services, and Imaging Services) had no one individual responsible for all three departments.
3) On 8/5/15 at 11:00 AM, Staff #1 and #2 were asked to provide Policies and Procedures for Outpatient Services. Staff #1 and #2 stated there were no Outpatient Services Policies and Procedures.
4) On 8/5/15 at 11:00 AM, Staff #1 and #2 were asked who was responsible for Outpatient Services. Staff #1 and #2 stated the three areas that provided Outpatient Services (Surgical Services, Laboratory Services, and Imaging Services) had no one individual responsible for all these Outpatient Service,

2) On 8/5/15 at 11:00 AM, Staff #1 and #2 were asked to provide Policies and Procedures for Rehabilitation Services. Staff #1 and #2 stated there were no Rehabilitation Services Policies and Procedures.
3) On 8/5/15 at 11:00 AM, Staff #1 and #2 were asked who was responsible for Rehabilitation Services. Staff #1 and #2 stated there was no individual responsible for Rehabilitation Services.
