1. Based on reviews of logs, record reviews, interviews, and reviews of policies and procedures - The hospital failed to review and resolve a known grievance levied by a patient's family.
The findings included:
a. A review of grievances was conducted in the conference room at 12:03 p.m. with the Director of Quality. An incident report dated 7/5/16 by the complainant related to injuries her father received on 7/3/16 following his bath was reviewed and revealed a brief description of the incident written by staff member S4, the person reporting the incident and receipt to the shift supervisor. The Director of Quality acknowledges the report that includes her signature.
b. During an interview with the complainant at 10:05 a.m. in room 319 of the hospital on 8/30/16 she claimed after the incident no staff member had contacted her and she did not believe the certified nursing assistant had reported the incident to his Charge Nurse. On 7/5/16 during her father's speech therapy session she informed the Therapist of the incident. The complainant submitted an actual complaint through the Texas Department of State Health Services via the Intake Hotline on 7/29/16.
c. Further interviews with the Director of Quality in the conference room following her own review of this incident report she acknowledged the nursing assistant should have initiated the process with the charge nurse to include notifying the patient's physician. She could not provide evidence at the time of this survey the incident had been investigated or reviewed in any other committees. She explained a report of suspected abuse and neglect should have required an investigation and resolution known to the complainant and family. Following her own review of hospital policies and procedures (see attached) Title" Abuse, Neglect, Assault Alleged or Suspected" policy # 001-01-035.9 revised Jul 2016 she acknowledged this grievance was not resolved and an investigation would be completed per Hospital policy and procedures.
