Based on observation and staff interview, it was determined that the facility failed to ensure that all ramp locations were provided with handrails, thus placing all persons that required a handrail at risk.
Findings were:
During the tour of the facility on 10/06/10 at 10:30 a.m. with staff M, it was determined that the ramps located at the main entrance were not supplied with a handrail on at least one side.
In interview with staff A and M on 10/06/10 at 12:30 p.m., the staff stated that a handrail could be installed on the ramps at the front entrance.

Based on observation and staff interview, it was determined that the facility failed to ensure that all components of the fire alarm system were properly monitored. This could place the facility at risk in the event of a failure of the phone line system.
Findings were:
During the tour of the facility on 10/06/10 at 12:15 p.m. with staff M, it was determined that the fire alarm systems phone line alarm could not be heard from a constantly attended location.
In interview with staff M on 10/06/10 at 12:20 p.m., he/she stated that the fire alarm company had already been scheduled to install an alarm at the fire alarm panel near the nurses station.

Based on observation and staff interview, it was determined that the facility failed to ensure that all required areas had sprinkler protection and that all sprinkler heads were being maintained. This could place all persons at risk in the event of a failure of the sprinkler system during a fire.
Findings were:
During the tour of the facility on 10/06/10 with staff M, the following sprinkler issues were noted:
1. Above the ceiling area of the patient wings and the kitchen areas at 11:00 a.m., observations revealed exposed wood framing around the perimeter of the building at the top of the outside walls and these areas were not protected by the sprinkler system; and
2. At 10:45 a.m., the sprinkler inspection report indicated that there were 4 sprinkler heads located in the Emergency Room (ER) area that had some paint on them and needed to be replaced.
In interview with staff M on 10/06/10 at 11:10 a.m., he/she stated that the exposed wood could be covered with an approved non combustible insulation and that the wood in question was part of the building's original construction and that there was no way to determine if the wood was fire retardant. Staff M also stated that the sprinkler heads in the ER area were already scheduled to be replaced.
