The inspector observed, while accompanied by the Plant Operations Manager during the hours of the inspection 10:30 AM to 2:00 PM on 04/17/2014 that that the facility fail to provide fire rated caulk for the penetration located at the smoke barriers walls at the following locations:
1) On the 1st floor above the ceiling of the smoke barrier cross corridor doors near the Electrical Room B
2) On the 2nd floor above the ceiling of the smoke barrier cross corridor doors near the Elevator Mechanical Room
3) On the 2nd floor above the ceiling of the smoke barrier cross corridor doors near the Room 226
This is not in accordance with NFPA 101, 2000: 19.3.7.3 and 8.3.6.

The inspector observed, while accompanied by the Plant Operations Manager during the hours of the inspection 10:30 AM to 2:00 PM on 04/17/2014 that there were smoke barrier cross corridor doors was not fully closed located at the following locations:
1) The smoke barrier cross corridor doors on 2nd floor near Room 228.
2) The smoke barrier cross corridor doors on 2nd floor near Room 209A.
This is not in accordance with NFPA 101, 2000: 19.3.7.6.

Based on observation the facility failed to provide adequate medical gas system storage requirement.
The inspector observed, while accompanied by the CEO and the Plant Operations Manager during the hours of the inspection 10:30 AM to 2:00 PM on 04/17/2014 that there were the following issues.
A.) The medical gas Oxygen cylinders stored within each smoke compartment were over 300 cf.

Facility failed to maintain essential electrical system wiring in accordance with the 1999 edition of the Standard for Health Care, NFPA 99. Findings included the following:
The inspector observed, while accompanied by the CEO and the Plant Operations Manager during the hours of the inspection 10:30 AM to 2:00 PM on 04/17/2014 that the power source of the power operated doors was not identified to be on Life Safety Branch electrical panel. Also the life safety electrical panel directory was not updated to include the power operated doors. This is not in accordance with NFPA 99, 1999: 3-4.2.2.2 (a) Life Safety Branch.
Facility failed to provide a battery powered lighting units at the essential automatic transfer switch location located at the Electrical Room A. This is not in accordance with NFPA 99, 1999: 3-4.2.2.2.(b)5. " The emergency generator location shall have task illumination; battery charger for emergency battery powered lighting unit(s), and selected receptacles at the generator set location and essential automatic transfer switch location "
