Based on observation and staff interviews it was determined the facility failed to ensure proper separation of the vertical opening stairway.
The findings include:
On 01/08/13 between 01:00 pm and 03:00 pm, observation revealed that the interior stairway was not properly separated at each of the three floor levels.
These findings were confirmed by Staff M at the time of discovery.

Based on observation and and staff interviews it was determined the facility failed to ensure complete sprinkler coverage is provided and that the sprinkler system is being properly maintained.
The findings include:
On 01/08/13 between 10:00 am and 12:00 pm, observation revealed that the following:
a) Sprinkler protection was not provided for the combustible over-hang at the service entrance.
b) Escutcheon plates were missing on numerous sprinkler heads.
c) An approved and properly supplied spare sprinkler head box shall be provided at the sprinkler riser.
These findings were confirmed by Staff M at the time of discovery.
