Based on observations made during the survey of the facility between the hours of 2:00 pm and 6:00 pm, while accompanied by the hospital's Administrator and Director of Support Services it was observed that the rquired 1 hour fire separation construction was missing in regards to the construction of the walls and the door opening for the CLEAN UTILITY ROOM.

Based on observations made during the survey of the facility between the hours of 2:00 pm and 6:00 pm, while accompanied by the hospital's Administrator and Director of Support Services it was observed that there was some recent documentation that the yearly inspections had taken place, but the facility failed to provide a log demonstrating a history for the yearly sprinkler inspection.

1. Based on observations made during the survey of the facility between the hours of 2:00 pm and 6:00 pm, while accompanied by the hospital's Administrator and Director of Support Services it was observed that the facility's log for ELECTRICAL EQUIPMENT INSPECTIONS - BIOMEDICAL failed to provide any indication for the frequency of the inspections in either the CRITICAL AREAS or the GENERAL AREAS.


2. Based on observations made during the survey of the facility between the hours of 2:00 pm and 6:00 pm, while accompanied by the hospital's Administrator and Director of Support Services it was observed that the facility failed to provide a log for ELECTRICAL POWER DISTRIBUTION AND GROUNDING SYSTEMS - EFFECTIVENESS.

3. Based on observations made during the survey of the facility between the hours of 2:00 pm and 6:00 pm, while accompanied by the hospital's Administrator and Director of Support Services it was observed that CLEAN STORAGE ROOM (near the ENDO TREATMENT ROOM) did not have the required 18 inches of clearance between the sprinkler head deflector and the top of storage items as required by NFPA 13, 1999: 5-5.6 "Clearance to Storage".

4. Based on observations made during the survey of the facility between the hours of 2:00 pm and 6:00 pm, while accompanied by the hospital's Administrator and Director of Support Services it was observed that OUTSIDE MED. GAS MANIFOLD ROOM had an electrical junction box at the north wall which was less than 60 inches above finish floor. This is not in compliance with the following:
NFPA 99, 2002: 5.1.3.3.2(5) " Locations for central supply systems and the storage of medical gases shall be compliant with NFPA 70 National Electric Code, for ordinary locations, with electrical devices located at or above 1520mm (5 ft) above finished floor to avoid physical damage. "
