Based on record review and interview, the facility failed to obtain weekly body weight measurements as ordered by a physician for 1 patient (#1) of 8 patients reviewed.
The findings included:
Review of the medical record revealed Patient #1 was admitted to the facility on 6/5/18 with diagnosis including Status Post Decompression Surgery, Cervical Spine Stenosis, and Diabetes.
Medical record of a physician's order dated 6/5/18 revealed "...Q 7 day Weight [measure body weight every 7 days]..." Further review revealed no weights were documented from 6/6/18 until the patient's discharge on 7/5/18 (4 weeks).
Interview with the Chief Executive Officer (CEO) on 7/25/18 at 10:00 AM, in the CEO's office, confirmed there was no documentation of the patient's weights from 6/6/18 to 7/5/18 and the facility failed to follow a physician's order.
