Based upon record review, staff interviews, and inspection of the fire site and smoking area with the facility security personnel on 1/10 & 11/ 2017 between approximately 0900 and 1545 hours the facility has failed to follow a written policy or regulation for control of residents smoking materials. This could result in the ignition of the combustible materials being brought back into the building.
The findings include, but are not limited to:
On November 30, 2016 at 2020 hrs a resident started a fire in the residents clothing wardrobe of the sleeping room igniting paper and clothing with a lighter that had been brought in from the smoking area, upon inspection of the smoking area locked mail box type storage it was observed that lighters and matches are being stored in the box's, this area is unsupervised by facility staff, upon returning to the building the residents are scanned for contraband by a metal detector, however the scanning was being done by staff that had not been trained to use the device thus possibly missing items coming into the building.
The above was discussed and acknowledged by the facility security person.
