Based on record review and interview, the hospital failed to list the Oklahoma State Department of Health as an entity for patients to contact to register a complaint.
Findings:
The surveyors reviewed a document titled "Patient Rights and Responsibilities", which included a section titled "Complaints, Concerns and Questions". There was no contact information for the Oklahoma State Department of Health included anywhere on the form.
On 07/01/22 at 11:55 a.m., Staff A stated the Oklahoma State Department of Health was not listed on the notice given to patients and the posted notices.
