Based on observation, it was determined the facility failed to maintain all components of the sprinkler system.
The findings included:
Observations during the re-certification survey on 6/17/13 at 9:45 AM revealed 1 of 1 sprinkler heads at the main entrance porch to the facility was corroded and in need of replacement.
Observations during the re-certification survey on 6/18/13 at 9:36 AM revealed damaged sprinkler heads at the following locations:
a. 1 of 2 at the nurses' station.
b. 1 of 1 at the nursing entrance.
These findings were verified by the maintenance director and acknowledged by the administrator at the exit conference on 6/18/13.

Based on record review and interview, it was determined the facility failed to maintain the emergency generator power supply.
The findings included:
Review of the facility's generator testing records on 6/18/13 at 11:15 AM in the maintenance director's office revealed the facility failed to document weekly checks on the water levels in the generator batteries.
Interview with the maintenance director on 6/18/13 at 11:20 AM in the maintenance directors office revealed the maintenance director was unaware of the requirement for weekly battery water level checks.
This finding was verified by the maintenance director and acknowledged by the administrator during the exit conference on 6/18/13.
