Based on observation, record review and staff interview, the facility failed to ensure all medical gas area alarms were functional. This condition would slow the notification to staff of an alarm condition within the medical gas system.
Findings are:
Observation on 8/24/21, from 10:30 am to 12:30 pm revealed the following medical gas area alarms were not functional: Minor OR, Major OR, PACU, ENDO, Patient Rooms 241-244.
Record review on 8/24/21 at 12:30 am revealed a quote dated 4/29/21 showed replacement panels were ordered, except for the Minor OR.
In an interview on 8/24/21, from 10:30 am to 12:30 pm, Maintenance A confirmed the panels were non-functional. Maintenance A provided the invoice, and stated the Minor OR panel was not ordered because that panel was not identified as non-functioning until the survey.
