Based on record review and interview, the hospital failed to ensure the protection of the patient's right to initiate a complaint with the OSDH for nine (Patients #1-7, 9 and #10) of 10 patients.
Findings:
Review of the medical record showed a document was provided to the patient which did not contain accurate contact information to file a complaint with the OSDH. Specifically:
1. A document titled "Cedar Ridge Hospital Complaint/Grievance Policy" read in part "You may contact the Oklahoma State Department of Health at (405) 271-6576. ADDRESS: 1000 NE 10th Street; Oklahoma City, OK 73117-1299"
Patient #1 signed the "Cedar Ridge Hospital Complaint/Grievance Policy" on 01/14/25.
Patient #2 signed the "Cedar Ridge Hospital Complaint/Grievance Policy" on 01/06/25.
Patient #3 signed the "Cedar Ridge Hospital Complaint/Grievance Policy" on 09/18/24.
Patient #4 signed the "Cedar Ridge Hospital Complaint/Grievance Policy" on 12/11/24.
Patient #5 signed the "Cedar Ridge Hospital Complaint/Grievance Policy" on 11/20/24.
Patient #6 signed the "Cedar Ridge Hospital Complaint/Grievance Policy" on 09/20/24.
Patient #7 signed the "Cedar Ridge Hospital Complaint/Grievance Policy" on 03/02/25.
Patient #9 signed the "Cedar Ridge Hospital Complaint/Grievance Policy" on 01/30/25.
Patient #10 signed the "Cedar Ridge Hospital Complaint/Grievance Policy" on 11/02/24.
The OSDH address and phone number of 123 Robert S. Kerr Ave, Oklahoma City, OK 73102 and (405) 426-8470 became effective November 2020.
On 03/13/25 at approximately 11:56 AM, Staff B reviewed the document titled "Cedar Ridge Hospital Complaint/Grievance Policy" and stated if patients tried to call or show up in person, they would not be able to reach OSDH. Staff B also stated they should periodically review the document.
