Based on record review, staff interview and policy and procedure review the facility failed to provide a copy of patient rights on admission to 11 of 11 sampled patients (Patients 1,2,3,4,5,6,7,8,9,10 and 11). This failure to provide patients a copy of the patient rights could affect all patients being cared for at the hospital.
Findings are:
A. Review of the policy and procedure titled, "Patient Rights and Responsibilities" (Effective: 1/5/23); revealed that:
3.5 All patients receiving care at Grand Island Medical Center shall receive the patient rights and responsibilities information.
3.6 Patient rights and responsibilities are available on www.giregional.org, provided to outpatients and emergency patients upon registration and to inpatients upon admission via handout.
An interview with the Quality Director on 4/13/2023 at 1:05 PM verified that the facility has not followed the policy and supplied the information to patients as stated in the "Patient Rights and Responsibilities" policy and procedure. "I was not aware that somehow got dropped."
B. Review of medical records revealed:
- Patient 1 admitted for Pulmonary Embolism (a blood clot in the lungs) on 3/3/2023 lacked verification of receipt of a copy of patient rights.
- Patient 2 admitted for Urinary Tract Infection on 4/3/2023 lacked verification of receipt of a copy of patient rights.
- Patient 3 admitted for hyponatremia (low sodium level) on 3/16/2023 lacked verification of receipt of a copy of patient rights.
- Patient 4 admitted for Tubular adenoma of colon (precancerous polyps in the colon) on 4/6/2023 lacked verification of receipt of a copy of patient rights.
- Patient 5 admitted for weight loss on 3/30/2023 lacked verification of receipt of a copy of patient rights.
- Patient 6 admitted for Pulmonary Embolism (a blood clot in the lungs) and shortness of breath on 4/11/2023 lacked verification of receipt of a copy of patient rights.
- Patient 7 admitted for Hematemesis (vomiting of blood) on 4/10/2023 lacked verification of receipt of a copy of patient rights.
- Patient 8 admitted for elevated liver enzymes on 4/11/2023 lacked verification of receipt of a copy of patient rights.
- Patient 9 admitted for urosepsis (severe infection of the urinary tract) on 4/12/2023 lacked verification of receipt of a copy of patient rights.
- Patient 10 admitted for fever on 2/12/2023 lacked verification of receipt of a copy of patient rights.
- Patient 11 admitted for hip fracture on 2/17/2023 lacked verification of receipt of a copy of patient rights.
C. Interview with Registrar-A on 4/13/2023 at 12:03 PM revealed that the registrars do not give a copy of patient rights and responsibilities to patients upon check in. Copies of condition of admission and privacy are supplied and available in 4 languages.
D. Interview with Quality Director on 4/17/2023 at 11:45AM revealed that on Friday 4/14/2023 staff researched why the facility hasn't given out patient rights and responsibilities. "The signs in admission are gone and the papers are gone. I am unsure when that happened."
