RN SUPERVISION OF NURSING CARE

Tag No.: A0395

Based on record review and interview, the hospital's registered nurse (RN) failed to supervise and evaluate the nursing care in that 1 of 1 patient (Patient #1) who filed a complaint to the Department did not have his PICC line (peripherally inserted central catheter) dressing changed from 8/15/15 to 9/4/15.

Findings included:

Patient #1 was admitted on 8/4/15 for "Sepsis with MRSA (methicillin-resistant staphylococcus aureus) and altered mental state" for 6 weeks IV (intravenous) antibiotic therapy and "rehabilitation if able to." The physician's order on 8/4/15 included dressing changes to "PICC line (peripherally inserted central catheter) every week...change caps with dressing change..."

Patient #1's medical record indicated there was no dressing changes performed from 8/15/15 to the time he was discharged on 9/4/15.

In an interview on 11/12/15 at 9:30 AM, Personnel #2 was informed of the above findings and confirmed the findings.

Policy and Procedure H-PC 08-044 "Dressing Change-Central Line/ PICC Line" revised 4/2014 required "To provide nursing personnel with guidelines to be used when changing dressings...8. All other central line dressings and catheter caps will be changed every 7 days. If the dressing required changing earlier than 7 days, the catheter caps will be changed as well."
