K51 Fire Alarm System
Based on visual observation the facility failed to assure that the fire alarm system was installed to provide effective warning to all parts of the building. The devices installed for the system ' s activation shall be by manual, detection or extinguishing operation which gives a sense of security to offer an advance warning in an emergency. This deficiency could potentially affect 32 of 32 residents.

Findings:

During the facility tour and the record review, between the hours of 9:30 AM and 4:00 PM on 8-25-16
It was observed that when the Fire Alarm was tested the Strobe Lights were out of Sync.

Interview with Maintenance Supervisor revealed the facility was not aware that the Strobe Lights were out of sync.

K75 Soiled Linen/Trash Receptacles
Based on visual observation the facility failed to provide mobile containers for dirty linen and trash collection that do not exceed the recommended capacity of 32 gallons. When hazardous materials are not stored in an area that is separated from other parts of the building, the risk of a fire/smoke emergency increases. This deficient practice could potentially affect 32 of 32 residents.

Findings:

During the facility tour, between the hours of 9:30 AM and 4:00 PM it was observed that the facility
Has carts that carry 2-32 gallon containers in corridors next to each other.

Interview with Maintenance Supervisor revealed the facility was not aware that the barrels were larger than 32 gallons.
