Based on record review and interviews, the hospital failed to ensure the discharge plan was developed by or under the supervision of a registered nurse, social worker, or other appropriately qualified personnel.
Findings:
Review of the hospital's policy titled "Discharge Planning and Transfers" effective date 05/2023, revealed in part:
Inpatient policy:
The Case Manager/Social Worker is responsible for initiating formal discharge planning upon admission of the patient.
Procedure:
2.) The RN Case Manager/Social Worker will work with the patient, family, patient's representative, or patient's physician to ensure their participations in setting patient care goals and the decision-making process.
Review of the hospital's staff list revealed S3CM as the only case manager employed by the hospital. Further review revealed there was no social worker listed as an employee of the hospital.
In an interview on 05/28/2024 during record review, S2DON verified S3CM was a licensed practical nurse.
In an interview on 05/28/2024 at 1:45 p.m. S3CM stated she was the only case manager for 32 patients.
