Based on document review and interview, nursing services failed to document repositioning every 2 hours for 1 of 10 patient medical records reviewed (P3).
Findings include:
1. Review of policy/procedure titled, "CORE: Clinical Guidelines for Pressure Injury," Policy number: H-PC 10-004, release date 06/2022; indicated under Policy: 5. Standard interventions for all patients can include but are not limited to: c. Repositioning orders (minimum every two-hour turns).
2. Review of P3's MR, vital flow sheet & daily ADL's, indicated lack of documented every two-hour entries of repositioning on the following dates: 01/16/25, 01/23/25, 01/24/25, 01/25/25, 01/28/25, 01/31/25, 02/09/25, and 02/12/25.
3. Interview with A2 on 03/03/25 at approximately 6:05 p.m., confirmed P3's MR lacked every two-hour entries for turns on the following dates: 01/16/25, 01/23/25, 01/24/25, 01/25/25, 01/28/25, 01/31/25, 02/09/25, and 02/12/25.
