Director of Quality & Risk Management and plant engineer, it was noted that recent inspection of doors in the means of egress, rated door assemblies and certain other doors had noted deficiencies.

Director of Quality & Risk Management and plant engineer the facility failed to provide documentation regarding grounding system for inpatient care areas and critical areas are being tested annually. Facility to initiate log and retain these records.
