Facility failed to maintain fire alarm system in accordance with 1996 edition of NFPA 72, National Fire Alarm Code. Findings included the following:

Surveyor observed between 9:00 and 10:00 am on July 28, 2015 that fire alarm box was not secured to wall in corridor near nurse station to prevent from damage in accordance with NFPA 72: 5-9.1.1.

Facility failed to maintain heating, cooling and ventilation system in accordance with the 2000 edition of the Life Safety Code, 1999 edition of NFPA 99, which references NFPA 90A and Ventilation Table for Hospitals and Outpatient facilities. Findings included the following:

Surveyor interview with administrator and maintenance staff and observations between the hours of 9:00 and 10:00 am on July 28, 2015 indicates that facility chiller was not functioning properly and portable fans were being utilized to resolve condensation issues in corridors of the hospital. The building HVAC equipment needs to be installed and maintained according to manufacturer ' s recommendations.

Facility failed to maintain medical air compressor in accordance with NFPA 99, Standard for Health Care Facilities. Findings included the following:

Surveyor interview with and administrator and maintenance staff and observations between 9:00 and 10:00 am on July 28, 2015 indicated that medical gas compressor was not functioning properly in accordance with NFPA 99: 4-3.1.1.9 Medical Air Compressor Supply Systems.
