Chapter 4 (NFPA 99)


Based on visual observation, the facility failed to assure a documented risk assessment was completed by a qualified personnel in accordance with NFPA 99. The risk assessment is required to be completed and on file at the facility. The deficient practice has the potential to affect 11 of 11 residents.

Findings:
During the complaint investigation on 5/3/22, between 9:30a-3:00p observation and record review revealed no documented risk assessment procedure.

Interview with the adminisrator revealed the facility was not aware the risk assessment was required.
