Based on review of facility documentation, staff interview and review of Emergency Department camera footage the facility failed to maintain a central log on each individual who comes to the emergency department, as defined in §489.24(b), seeking assistance and whether he or she refused treatment, was refused treatment, or whether he or she was transferred, admitted and treated, stabilized and transferred, or discharged.
Findings include:
In a review of documentation provided by the hospital CEO revealed that at least two patients presenting to Chillicothe Hospital Emergency Room were not added to the hospital ER log as required by 489.24(b),
· "In December I (CEO) fell and hit my head, knocked myself out and sustained a laceration to my left forehead. I was transferred to Wichita Falls to rule out a brain bleed When they called the Sheriffs office to get me transferred by EMS, they told the doctor that the hospital was on divert nevertheless I did get transferred. If you will note my name is not listed on the ER log book..."
· "August 5, 2018, Patient Steve Cheek: A patient by the name of Steve Cheek came to Chillicothe Hospital to the ER. This patient was not put into the EHR (hospitals electronic medication administration record) nor were they listed in the ER log book. The patients family member Susanna Belcher RN, works for Chillicothe Hospital and was told by Cheek that he was turned away from the hospital and not treated in the ER. Susanna Belcher, RN came to me within 3 days and informed me along side Dr. Wolf stating that her father-in-law came to the ER and he did not get seen. Dr. Wolf stopped and said, Stormy called me on that patient and I gave her an order to give the patient a shot of Toradol 30 mg IM (intramuscular). The patient never received the injection."
