Based on records reviewed and interviews the Hospital failed to consistently adhere to the Hospital's policy for advance directives in one of ten patients (Patient #2).

The Surveyor reviewed Patient #2's medical record on 6/6/17. The medical record indicated that Patient #2 had experienced an acute illness and was listed as a full code (if a patient has a cardiopulmonary arrest, the medical team will do everything in their power to prevent death) in the Progress Notes. However, the bright pink paper in the medical record titled "Code Status Order", indicated that Patient #2 was not to be resuscitated.

The Surveyor interviewed Registered Nurse #1 at 2:45 P.M. on 6/6/17. Registered Nurse #1 said the pink sheet was used as a quick reference for the nurse caring for the patient to determine the code status. Registered Nurse #1 said she was confused that the quick reference sheet was different from the Physician's orders and the Progress Notes. Registered Nurse #1 said the quick reference sheet would be the first place the bedside nurse would look in an emergency to determine a patient's code status.

Based on observations, records reviewed and interviews the Hospital failed to consistently provide for patient safety related to expired equipment and an unsecured/unsupervised medication.

1.) The Surveyor toured the patient unit on the second floor of the Hospital at 8:30 A.M. on 6/6/17. During a review of the emergency equipment the Surveyor noted that the defibrillator pads (used to create conductivity between the electrical current from the defibrillator machine and the patient) had expired in May 2017. The monthly Code Check List sheet indicated the equipment was checked on 6/2/17 and the expired pads were not removed or replaced.

2.) The Surveyor entered Room 221 during the tour of the second floor. An unsecured/unsupervised admixture of antibiotic had been left at the patient's bedside. The Nursing Supervisor said she believed the patient had left the Hospital and the room was in the process of being cleaned.
