Based on document review and interview, the facility failed to follow policy regarding discharge planning, in 1 of 5 (Patient 1) medical records reviewed.
Findings include:
1. Review of policy titled: DISCHARGE PLANNING, reviewed 01/2022,(by facility) indicated - the discharge plan begins day of admission and should prepare the patient and family for the transition to the next level of care, address patient and families need for instructions about continued treatment.
2. Review of patient 1's medical record (MR) indicated family member FM 1 was listed as Power of Attorney (POA) for this patient and the MR lacked documentation of family involvement and patient involvement in treatment plan.
3. Interview with S3 (Director of Clinical Services) on 04/14/2022 at 12:30 PM confirmed patient 1 lacked documentation of family involvement during patient 1's stay.
