Facility failed to maintain exiting from one door to Pharmacy in accordance with the 2000 edition of NFPA (National Fire Protection Association) 101. Findings included the following:

Surveyor observed between 9 and 10 am on 9/28/11 that door to corridor from pharmacy had deadbolt and slide bolt in addition to door latch on egress side. This is not in accordance with NFPA 101, 2000: 7.2.1.5.4. Doors shall be operable with not more than one releasing operation.

Facility failied to maintain electrical system in accordance with 1999 edition of the Standard for Health Care Facilities, NFPA 99. Findings included:

Surveyor observed between 10 and 11am on 9/28/11 that facility records did not include test of patient care electrical receptacles in the last year in accordance with NFPA 99: 3-3.3.3 Receptacles shall be tested in Patient Care areas as follows:

Phyisical integrity of each receptacle (visual)
Continuity of the grounding of each receptacle (test)
Correct polarity of hot and neutral connections in each receptacle (test)
The retention force of the grounding blade shal be not less than 4 ox (test)

Testing intervals:
General care areas tested annually
Critical care areas tested semi-annually
wet location tested annually
