Based on observation and interview, the facility failed to ensure doors to all patient rooms closed securely in the door frame to prevent the passage of smoke. This deficient practice affects all patients within that smoke compartment. The facility census was 46.

Findings included:

Observation during a tour of the facility, conducted on the afternoon of 03/31/15, showed the following patient room doors would not completely close in the door frames to prevent the passage of smoke.

*Observation at 1:24 PM showed the door to patient room 310 would not close completely in the door frame when tested.
*Observation at 1:25 PM showed the door to patient room 304 would not close completely in the door frame when tested.
*Observation at 2:30 PM showed the door to patient room 232 would not close completely in the door frame when tested.
*Observation at 2:43 PM showed the door to patient room 223 would not close completely in the door frame when tested.
*Observation at 2:54 PM showed the door to patient room 212 would not close completely in the door frame when tested.

Staff JJ, Director of Plant Operations, confirmed at that time the doors would not completely close in the door frames.

Section 19.3.6.3 of the National Fire Protection Association (NFPA 101) states doors shall be of construction to resist the passage of smoke.

Based on observation and interview, the facility failed to ensure one of four stairwell doors on the third floor will completely close and latch after being opened. This deficient practice affects all patients and staff on the third floor. The facility census was 46.

Findings included:

Observation at 1:35 PM on 03/31/15 during a tour of the facility, showed the self-closing mechanism attached to the door to the east stairway on the third floor did not close and latch the door securely in the door frame when manually tested.

Staff JJ, Director of Plant Operations, confirmed at that time the door would not completely close and latch in the door frame.

Section 19.3.1.1 of the National Fire Protection Association (NFPA 101) states any vertical opening shall be enclosed or protected in accordance with 8.2.5. Where enclosure is provided, the construction shall have not less than a 1- hour fire resistance rating.
