Based on a review of facility documents, medical records (MR), and interviews with staff (EMP), it was determined that the facility failed to provide Medicare beneficiaries with an Important Message from Medicare (IMM) within two calendar days of admission and within two days prior to discharge from the facility for six of ten medical records. (MR2, MR3, MR6, MR7, MR9, and MR10)

Findings include:

Review of "Notice of Non Coverage- Medicare Beneficiaries" last reviewed January 2018, revealed, "...Policy: All Hospitals are required to deliver the Important Message from Medicare(IM), CMS-R 193 to all Medicare beneficiaries who are hospital inpatient upon admission and prior to discharge..."

Review of MR2 revealed that the patient was admitted on February 7, 2018 and discharged on discharged on February 11, 2018. The review of the medical record did not reveal documentation that the patient received the first or second notice of the IMM.

Review of MR3 revealed that the patient was admitted on January 26, 2018. The patient received the first notice of IMM on January 29, 2018, three days after admission.

Review of MR6 revealed that the patient was admitted on January 12, 2018. The patient received the first notice of IMM on January 15, 2018, three days after admission. The patient was discharged on February 5, 2018, and received the second notice on February 2, 2018, three days prior to discharge.

Review of MR7 revealed that the patient was discharged February 10, 2018. Further review of MR7 revealed the patient received the second IMM on February 6, 2018, four days prior to discharge.

Review of MR9 revealed that the patient was admitted on February 2, 2018. Further review of MR9 revealed the patient received the IMM on February 6, 2018, fours days after admission.

Review of MR10 revealed that the patient was admitted on January 12, 2018. Further review of MR10 revealed the patient received the IMM on January 15, 2018, three days after admission.

An interview conducted on March 5, 2018, at 10:30 AM, confirmed the IMM's were not given timely.
