Based on record review and interview, the hospital failed to ensure assessments were documented in alignment with the restraint policy for 1 (Patient #20) of 13 restraint medical records reviewed.

This failed practice had the potential to delay recognition of changes in patient's condition to determine if restraints remained necessary.

Findings:

A hospital policy titled "Restraints and Seclusion" stated in part to include removal of restraints at least 10 minutes every 2 hours or more often.

Patient #2 medical record showed on 04/09/2024 at 6:00 PM, restraints were not removed and assessed per policy. On 04/11/2024 Staff A stated this policy was not completed. The medical record also showed no documented removal of restraints.
