Based on observation, it was determined that the facility failed to maintain the doors protecting the corridor.

The findings included:

Observation on 12/5/11 at 10:30 AM, revealed the respiratory therapy treatment room door would not close within the door frame.

This findings was acknowledged by the administrator and director of maintenance at the exit conference on 12/5/11.

Based on observation the facility failed to maintain the exit signs.

The findings included:

Observations on 12/5/11 at 10:45 AM, revealed the corridor leading to the exit was missing the exit sign.

This findings was acknowledged by the administrator and director of maintenance at the exit conference on 12/5/11.

Based on observations, it was determined that the facility failed to maintain the medical gas system.

The findings included:

Observation on 12/5/11 at 10:14 AM, revealed above the ceiling adjacent to room 208 the medical gas lines were not isolated and laying on dissimilar metal.

This finding was acknowledge by the administrator and director of maintenance at the exit conference on 12/5/11.

Based on testing, it was determined that the facility failed to maintain the electrical equipment.

The findings included:

Observations on 12/5/11 at 12:20 AM, revealed in bathroom of room 201, the ground fault circuit interrupt failed during testing.

This finding was acknowledged by the administrator and director of maintenance during the exit conference on 12/5/11.
