The hospital environment failed to be maintained in a manner that the safety patients was assured.
Based on observation during a tour of the hospital with the nursing director the floors in 4 of 5 operating rooms (OR) failed to be intact, the walls were observed with plaster visible and metal equipment was rusted.
Findings were:
The floor in OR #1 was observed to have at least 11 cuts and broken areas, the largest 1"x 1/2";
the floor in OR #2 also had multiple small cuts and broken areas; the floor in OR #4 had multiple cuts and small holes; the floor in OR #5 had over 30 cuts and broken areas - the floors are not intact, cannot be cleaned and provide a source for bacterial growth.
The walls in OR's 1, 2, 4, and 5 were not intact and plaster was visible. The walls cannot be cleaned and the open plaster is a source of bacterial growth.
There were four poles (that hold intravenous fluids and lines) observed that had the bases cracked and peeling, not being cleanable and a possible source of infections.
There were three medication carts observed to have rusty handles; these carts are used by staff with gloves in the operating rooms who also provide patient care, providing a source of contamination.
In both the preoperative area and post operative area there were large areas of the floor that were cracked and crumbling , the area was not smooth, and could not be cleaned with all of the open areas, this also provides a source of bacterial growth.
The above was confirmed in interviews with the nursing director on 6/20 and 21 in the surveyors work area.

Based on review of monitoring records from the operation of the sterilizers in the facility for 2012, the facility failed to ensure that the records included the contents of each load sterilized as required.
Findings were:
Review of the sterilizer run records for 2012 revealed that the contents of each load failed to be documented, and therefore not able to be traced if an infection had occurred in a patient in the facility.
Interview with the surgery director and nursing director in the surveyor work area on 6/21/12 confirmed the above findings.
