Based on visual observation the facility failed to provide corridor doors that were latching in the frame. When the doors latch a smoke resistive seal is formed to protect the room ' s occupants. The deficient practice had the potential to affect 7 of 7 residents.
1 of 2 corridors had doors that were deficient.
Findings:
During the facility tour on 2/14/2018, between the hours of 8:00a-4:30p observation revealed the door to the nurse lounge did not latch in the frame. Observation also revealed an open vent in the bottom of the door.
Interview with Administrator revealed the facility was not aware of the door to the nurse lounge was not latching in the frame and that the door did not create a seal, when closed, to prevent the transfer of smoke.

Based on visual observation, the facility failed to comply with the requirements of NFPA 99 and other applicable codes with regard to electrical systems. In cases of a failure in the electrical systems this could have a detrimental affect on the life safety of the occupants. The deficient practice had the potential to affect 7 of 7 residents.
Findings:
During the facility tour on 2/14/2018, between the hours of 8:00a-4:30p observation revealed the hydroculator located in the therapy room was not connected to a GFI outlet. Observation also revealed the medicine refrigerator was not plugged into an outlet that was designated on eemergency power.
Interview with Administrator revealed the facility was not aware that these electrical system components were not provided.

Based on visual observation, the facility failed to provide a remote annunciator to operate outside of the generating room in a location readily observed by operating personnel. The annunciator is hard-wired to indicate alarm conditions of the emergency power source. In cases of a power outage the emergency generator powers essential life safety equipment for the facility. The deficient practice had the potential to affect 7 of 7 residents.
Findings:
During the facility tour on 2/14/2018, between the hours of 8:00a-4:30p observation revealed that there was no remote annunicator installed for the generator.
Interview with Administrator revealed the facility was not aware that a remote annunciator was not installed to indicate alarm conditions of the emergency generator.
