Based on visual observation the facility is using the electrical room for storage affecting 1 of 2 smoke compartments. This deficient practice if not corrected could cause harm to 8 of 28 residents.
Findings: During tour of building on 8-8-2011 at 11:00 a.m the electrical closet was observed storing maintenance items which could block the electrical panels, also the room is not 1 hour fire rated. NFPA 101 requires storage areas to be fire rated with self closing doors. NFPA 70 prohibits storage in electrical rooms.

Based on visual observation and record review the facility failed to have a current fire alarm inspection affecting 2 of 2 smoke compartments. This deficiency if not corrected could have the potential to cause harm to the 28 patients in the building.
Findings: During tour of building on 8-8-2011 review of records revealed the facility failed to assure the fire alarm system was maintained in a manner to assure activation in case of a fire emergency. Records indicated the last inspection performed on the fire alarm system by a licensed contractor was dated 6-7-2010. There were no other records to review. NFPA 72 requires annual inspections of systems performed by licensed contractors.

Based on visual observation the facility failed to provide adequate sprinkler protection in the kitchen and the activity/therapist room, affecting 1 of 2 smoke compartments. This deficiency if not corrected could have the potential to cause harm to the 28 patients in the building.
Findings: During tour of the building on 8-8-2011 at 11:00 a.m. and confirmed by the administrator the activity/therapist room and the kitchen entrance and kitchen were observed not having adequate sprinkler protection because of a wall that was erected to separate the activity area from the kitchen. The wall is partially covering two of the sprinkler heads which obstructs coverage to both the activity room and the kitchen, on the opposite side of the wall the entrance into the kitchen has no coverage because of the wall obstruction

Findings: During tour of building on 8-8-2011 review of records revealed the facility failed to assure the sprinkler system was maintained in a manner to assure activation in case of a fire emergency. Records indicated the last inspection performed on the sprinkler system by a licensed contractor was dated 6-10-10. There were no other records to review. NFPA 13 requires annual inspections of systems performed by licensed contractors.
