Based on observation, it was determined that the facility failed to ensure that Alcohol Based Hand Rub dispensers (ABHR) are not installed above ignition sources.
Findings include:
On 2/15/12 at 10:30 AM, in the presence of Staff #1, it was observed that an ABHR was installed above an electric light box in the corridor at Room #234.
