Facility failed to maintain HVAC systems in accordance with NFPA 90A, Standard for Air Conditioning Systems. Findings included the following:
Surveyor observed on 12/14/11, between 11:00 and 11:30 am, that ventilation from the pharmacy was negative relative to the corridor. This does not meet ventilation table which requires a positive ventilation is relation to the corridor for pharmacies.

Facility failed to maintain kitchen hood protection system in accordance with Standard for Kitchen Equipment, NFPA 96. Findings included the following:
Surveyor observed on 12/14/11, between 10:30 and 11:00 am that in the kitchen access to natural gas disconnect for kitchen hood fire protection system was blocked by dish cart.

Facility failed to maintain electrical system in accordance with the 1999 edition of the Standard for Health Care Facilities, NFPA 99. Findings included the following:
Surveyor observed on 12/14/11, between 11:00 and 11:30 am as follows:
1. Medical gas alarm panel was indicated to be powered by circuit #13 in electrical panel 1LEQ1, Equipment Branch. This is not in accordance with NFPA 99, 3-4.2.2.2. Medical gas alarms are to be powered by the Life Safety Branch of the emergency system; and
2. Electrical panel 2LLS1, circuit #3 powered a isolation monitor. This is not in accordance with NFPA 99: 3-4.2.2.2. is not included within the itmes which may be powered by the the Life Safety Branch of the emergency system.

Facility failed to mantain electrical receptacle testing logs in accordance with NFPA 99, Standard for Health Care Facilities. Findings included the following:
Surveyor observed on 12/14/11, between 11:00 and 11:30 am that facility records did not include results of
testing receptacles in accordance with NFPA 99: 3-3.4.2.3. Receptacles shall be tested in Patient Care areas as follows:
Phyisical integrity of each receptacle (visual)
Continuity of the grounding of each receptacle (test)
Correct polarity of hot and neutral connections in each receptacle (test)
The retention force of the grounding blade shal be not less than 4 oz. (test)
Testing intervals:
General care areas tested annually
Critical care areas tested semi-annually
wet location tested annually.
