Based on observation and interview, the facility failed to ensure 17 of 17 outdated Dextrose 5% 100 milliliters (ml) intravenous solutions located in the Intravenous Prep Room of the Pharmacy were not available for the preparation of medications to be administered to patients. Findings follow:

A. A tour of the Pharmacy was conducted on 01/15/14 at 1430. Seventeen of a total of seventeen Dextrose 5% 100ml intravenous solutions were observed in the Intravenous Prep Room of the Pharmacy. All of the Dextrose 5% 100ml outdated 11/30/13.

B. An interview was conducted with the Director of Pharmacy at 1435 on 01/15/14. It was verified through interview the solutions were out of date.

REGULAR FIRE AND SAFETY INSPECTIONS

Based on interview it was determined the facility did not ensure regular inspections by the local fire control Agency due to a lack of documentation available for review. The failed practice had the potential to affect the health and safety of all patients, staff and visitors because the Fire Department familiarity of the potential hazards and physical layout of the facility was not assured. The facility had a census of 10 patients on 01/14/14. The findings follow:

In an interview on 01/15/14 at 1300 the Facilities Manager verified there was no documentation of Fire Department inspection available for review.
