Based on document review & interview the facility failed to maintain a license to operate a Psychiatric Inpatient Facility (see tag A0022).
The cumulative effect of this systemic problem resulted in the hospital's inability to ensure compliance with State Laws were followed.

Based on document review & interview the facility failed to maintain a license to operate a Psychiatric Inpatient Facility as required by Indiana Code (IC) 12-25-1-3 for 1 facility.
Findings include;
1. Review of IC 16-21-10-4 indicates the following;
Sec. 4. (a) As used in this chapter, "hospital" means either of the following:
(2) A private psychiatric hospital licensed under IC 12-25.
2. Review of IC 12-25-1-3 indicates the following;
Sec. 3. A person must hold a license issued by the secretary or the secretary's designee
to establish, conduct, operate, or maintain a private institution under any name for the
treatment and care of individuals with psychiatric disorders, developmental disabilities,
convulsive disturbances, or other abnormal mental conditions.
3. On 5/31/2018 at 1530 hours staff #40 (Assistant Deputy Director for the Office of Division of Mental Health Agency) confirmed the facility's license to operate as a Psychiatric Inpatient Facility was not being renewed and the license would expire on 5/31/2018 at midnight.
4. On 6/01/2018 at 1335 hours staff #40 confirmed the facility's license expired to operate as a Psychiatric Inpatient Facility.
