Based on observation, the facility failed to maintain one hour fire resistance rating on the smoke barrier wall.
Findings include:
At 10:30 a.m., on December 7, 2011, it was observed that a penetration (approximately 2" diameter) existed above the sprinkler piping in the smoke barrier wall over the janitor closet next to the back dock. The maintenance person made this observation during the survey process.
