Based on observations during the facility walk through survey on 03/26/2015, with the plant engineer, it was noted that a set of cross-corridor 1 1/2 hr. "B" rated fire doors lacked astragals along the meeting edge of the doors, and they did not close completely when tested with the fire alarm system.

Based on observations during the facility walk through survey on 03.26.2015, with the plant engineer, it was noted that there were no emergency generator remote annunciator panels located anywhere within the hospital as reqired. NFPA 99 (2002) requires that a remote annunicator panel be located at the site of the engineering/maintenance staff office or work area. And if that is not staffed 24 hours an additional annunciator panel shall be located at a 24 hour staffed location, such as a nurse station or PBX operator.
