Based on observations the facility failed to provide smoke barriers that were constructed with at least a one-half hour fire resistance rating for 1of 3 walls. This deficiency affects 2 of 3 smoke compartments and 15 residents in the facility.
Note: NFPA 101, 2000 edition:
NFPA 101:19.3.7.3 Any required smoke barrier shall be constructed in accordance with Section 8.3 and shall have a fire resistance rating of not less than 1/2 hour.
Findings:
During facility tour on August 17, 2011between 1:00 p.m. and 3:00 p.m., the integrity of a smoke barrier wall was observed to be compromised in the following area.
1. A hole 8 ? inches by 8 inches was observed in the smoke barrier wall above the suspended ceiling across from the Medication room that is not sealed.

Based on observation the facility failed to provide a clear and unobstructed access to the exit door for 1 of 4 stairwells in the facility. This deficiency affects 22 patients on the 9th floor of the facility.
Note: NFPA 101, 2000 edition
NFPA 101: 7.5.1.1 Exits shall be located and exit access shall be arranged so that exits are readily accessible at all times.
Findings:
During facility tour on August 18, 2011 between 8:00 a.m. and 11:30 a.m. there was a table and 2 chairs blocking the access of the #2 stairwell on the 9th floor. This could prevent the rapid removal of patients in case of a fire or disaster.
