Sprinkler System - Installation

Based on observation and staff interviews it was determine the facility failed to ensure that the facility was completely sprinkled.
This could place 30% residents at risk in the event of an emergency.
The findings include:
During a tour of the facility with Staff M on 02-06-19 between 10:00 am and 3:00 pm observation revealed that shower room in Zone 2 was missing two sprinkler heads.
These finding were confirmed by Staff M at the time of discovery.

Based on observation and staff interviews it was determine the facility failed to ensure that Smoke door Zone 3 failed to close completely.
This could place 50% residents at risk in the event of an emergency.
The findings include:
During a tour of the facility with Staff M on 02/06/19 between 10:OO am and 3:pm observation revealed that Smoke Door Zone 3 by rooms 113 and 114 failed to seal and latch properly when tested
These findings were confirmed by Staff M at the time of discovery.
