Based on document review and interview the facility failed to ensure a physicians order was present for intravenous catheter placement for one (1) of ten (10) medical records(MR) reviewed (P # 10).

Findings include:

1. Review of P # 10's MR indicated on 4/08/23 at 5:00 pm an IV was first assessed for necessity, line status, site condition, dressing type, dressing condition and patency. Review of MR lacked documentation of a physician's order for an IV.

2. In interview on 6/13/2023 at approximately 12:00 pm with administrative staff member A # 6 (Nursing Professional Development Manager), confirmed that there should have been an order in place from the physician for an IV.
