Based on policy review, record review and interview the facility failed to follow its policy to inform family/patient of the results of their grievance investigation in 11 of 11 patient medical records reviewed (#'s 1, 2, 3, 4, 5, 6, 7, 8, 9, 10 and 11).

On 2/12/2013 at 1:00 PM in the conference room, facility policy # CS1-14 Concern/Complaint/Grievance Policy, was reviewed and revealed the following on page 4, under the "Timely Response" section: the written response will include the name of the hospital contact person, the steps taken on behalf of the patient to investigate the grievance, the results of the grievance investigation and the date of completion.

On 2/12/2013 in the conference room the grievance log was reviewed and grievances filed by patients #1, 2, 3, 4, 5, 6, 7, 8, 9, 10 and 11 were reviewed, 11 of 11 letters of notification did not include the steps taken on behalf of the patient to investigate the grievance, the results of the grievance investigation or the date of completion.

An interview with staff #2 confirmed this information was not released in the notification letters.
