Based on observation the facility failed to provide adequate protection for existing rated walls and smoke walls.
"Penetrations for cables, cable trays, conduits, pipes, tubes, combustion vents and exhaust vents, wires, and similar items to accommodate electrical, mechanical, plumbing, and communications systems that pass through a wall, floor, or floor/ceiling assembly constructed as a fire barrier shall be protected by a firestop system or device." - NFPA 101, 2012, 8.3.5.1.

Based on observation a general comment to the facility; verify all equipment and circuits on essential sytem are properly identified as required by NFFPA 70.
Identification. All boxes and enclosures (including transfer switches, generators, and power panels) for emergency circuits shall be permanently marked so they will be readily identified as a component of an emergency circuit or system." - NFPA 70, 2011, 700.10.
Based on observation a the facility failed to proper covers for junction boxes above ceiling. As a maintanance schedule facility should verify that all junction boxes are properly covered.
"Covers. All pull boxes, junction boxes, and conduit bodies shall be provided with covers compatible with the box or conduit body construction and suitable for the conditions of use. Where used, metal covers shall comply with the grounding requirements of 250.110." NFPA 70, 2011, 314.28 (C)
Based on observation a the facility failed to properly secure existing low voltage wires that are resting on either ceiling grid or panels.
Wiring shall comply with NFPA 70, 2011.
"Secured in Place. Raceways, cable assemblies, boxes, cabinets, and fittings shall be securely fastened in place. Support wires that do not provide secure support shall not be permitted as the sole support.
