Based upon observation and interview, it was determined the facility failed to maintain the proper building construction type for a three-story building in one hundred percent of the facility.
Findings include:
1. Observation on June 9, 2014, revealed the following building construction type deficiency:
a) At 9:15 a.m., the building construction is a three-story with basement Type III (200) unprotected ordinary construction building that is fully sprinklered. This construction type is not permitted as per NFPA 101 19.1.6.2.
Interview with the Facility Administrator and Maintenance Director on June 9, 2014, at 2:45 p.m., confirmed the building construction type issue.

Based upon observation and interview, it was determined the facility failed to maintain smoke barrier doors in two instances on one of four floors.
Findings include:
1. Observation on June 9, 2014, revealed the following smoke barrier door deficiencies:
a) At 10:04 a.m., the smoke barrier door to room 258 on the second floor in the Wilkins building did not close and latch in its frame.
b) At 10:30 a.m., the smoke barrier door to the second floor gymnasium storage room was being held open with a door wedge and lacked an automatic closing device on one leaf of the door.
Interview with the Facility Administrator and Maintenance Director on June 9, 2014, at 2:45 p.m., confirmed the smoke barrier door issue.

Based upon observation and interview, it was determined the facility failed to maintain exit access requirements in one instance on one of four floors.
Findings include:
1. Observation on June 9, 2014, revealed the following exit access deficiency:
a) At 10:23 a.m., the emergency exit door in the second floor gymnasium was being blocked by a swing, gym mats, and other equipment.
Interview with the Facility Administrator and Maintenance Director on June 9, 2014, at 2:45 p.m., confirmed the exit access issue.

Based upon observation and interview, it was determined the facility failed to properly install an automatic sprinkler system in one instance on one of four floors.
Findings include:
1. Observation on June 9, 2014, revealed the following automatic sprinkler system deficiency:
a) At 12:35 p.m., there was a light fixture obstructing a sprinkler head in the second floor kitchen dish room. The light fixture cover was altered to fit the sprinkler head inside of the fixture which obstructs the sprinkler from providing adequate coverage to the area.
Interview with the Facility Administrator and Maintenance Director on June 9, 2014, at 2:45 p.m., confirmed the automatic sprinkler system issue.

Based upon observation and interview, it was determined the facility failed to maintain the automatic sprinkler system in one instance on one of four floors.
Findings include:
1. Observation on June 9, 2014, revealed the following sprinkler maintenance deficiency:
a) At 12:38 p.m., there were three sprinkler heads in the second floor kitchen dish room that had paint covering the sprinkler deflector. The paint covering the deflector prohibits proper deflection of the sprinkler when activated.
Interview with the Facility Administrator and Maintenance Director on June 9, 2014, at 2:45 p.m., confirmed the sprinkler system maintenance issue.

Based upon observation and interview, it was determined the facility failed to maintain fire extinguisher access requirements in one instance on one of four floors.
Findings include:
1. Observation on June 9, 2014, revealed the following fire extinguisher access deficiency:
a) At 10:27 a.m., the fire extinguisher was inaccessible in the second floor gymnasium. There were several bikes and wheel chairs blocking access to the fire extinguisher.
