LIFE SAFETY FROM FIRE

Based on observations and staff interviews, the facility failed to meet the provisions of the Life Safety Code related to exit directional signage, oxygen storage, oxygen storage signage, and the use of power strip cords. This could affect all patients in the facility. The total census during this visit was 29 patients.

Findings include:

During this visit, on 02/25/13 between 1:30 PM and 4:10 PM, tour was conducted in the facility with Staff B, D, and E. During this tour the following areas were observed:

The second floor gymnasium lacked exit directional signage for one of three exits. Refer to A0710.

The facility failed to maintain less than 300 cubit feet of oxygen in one smoke compartment. Refer to A0710.

The facility failed to ensure an "oxygen in use sign" was posted on one oxygen storage room. Refer to A0710.

The A East wing nurses' station was observed with two power strips in use, which were plugged into each other, and then into the wall, which was against manufacturer's guidelines. Refer to A0710.

The facility failed to maintain less than twelve E-tanks of oxygen in a smoke compartment. Refer to A0710.

This could affect all patients in the facility. The total census during this visit was 29 patients.
