INFECTION CONTROL PROGRAM

Based on observation, record review and interviews, the hospital failed to have a system in place to prevent and control the transmission of infections by failing to maintain a sanitary hospital environment.
Findings:

1) On 04/07/21 at 11:00 a.m., observation of the patient care areas revealed the following:
The bathroom shared by double occupancy rooms A and B had a black substance coating the surface in the corners of the shower stall, and there was no bath tissue available near the toilet; in room A, bed b had a T-shirt under the mattress; room B beds had bits of paper and plastic items underneath the mattresses; all 4 beds in rooms A and B were without bed linens and trash was observed on the floors.

Double occupancy rooms C and D had trash and debris on the floors and underneath the mattresses of the unmade beds; the bathroom doors (soft padded doors with Velcro attachment) were lying on the floor, with trash observed in the trash cans and a used shampoo bottle sitting in the shower stall.

Review of the list of discharges for the period dated 03/12/21-04/06/21 revealed the most recent discharges from the above rooms as follows: Room A, bed a - no documented discharge during this period; Room A, bed b - most recent discharge was 03/19/21; Room B, bed a - most recent discharge was 03/04/21; Room B, bed b - most recent discharge was 03/02/21; Rooms C and D - no documented discharges during this period.

On 04/07/21 at 11:45 a.m., interview with S1Director of Quality and S2Housekeeping Supervisor confirmed that there was no evidence that the above rooms had been cleaned since the last discharged patients left, and that there was no consistent system in place for communication between the nursing staff and housekeeping staff to ensure that rooms were cleaned promptly upon discharge and made ready for new admissions.

2) On 04/07/21 at 11:15 a.m., interview with S1Director of Quality revealed that ICU room E was clean and ready for a new patient. At that time, observation of ICU room E revealed the IV pole was covered with a clean trash bag (indicating clean) and clean linen was on the bed. Further observations revealed red/brown splatters on the wall behind bed, old tape stuck to foot of bed, a brown substance on the call bell and the privacy curtain had a brown substance on it. At this time, interview with S1Director of Quality confirmed the room was in need of further cleaning.
