Facility failed to maintain sprinkler system in accordance with of the Standard for Health Care Facilities, NFPA 99. Findings included:
Surveyor observed between 9:30 and 10:00 am on 01/06/2011 that inspector test valve was not identified properly. This does not meet the requirements of NFPA 13, 1996: 2-7.3 with Appendix A-2.7.4. An "Inspector's Test Valve" sign must be provided and attached to the inspector test valve secured with noncorrosive wire, chain, or other means.

Facility failed to maintain medical gas storage area in accordance with the Standard for Health Care Facilities, NFPA 99. Findings incuded:
Surveyor observed between 9:30 and 10:00 am on 01/06/2011 that medical gas bottles were not properly secured in racks. All medical gas cylinders, in use or empty, shall be fastened to racks or otherwise secured in place to prevent damage to the cylinders. Empty cylinders shall be segregated from full cylinders, when stored in the same enclosure, and shall be marked to avoid confusion or delay if a full cylinder is needed in a hurry - NFPA 99, 1999: 4-3.5.2.2.
