Based on observation the facility failed to ensure that all doors positively latched and remain closed for one patient room door. The census was 92.
Findings included:
During tour on 05/17/11 at 9:50 AM the door to room 316 was closed and the latch did not keep the door closed and the door could easily be pushed open. There were two patients observed to be in the room at the time.

Based on observation and record review the facility failed to ensure that three of three smoking areas for patients have self closing metal containers for disposal of smoking materials. The census was 92.
Findings included:
Observation on 05/17/11 revealed that there were three outdoor smoking areas used by patients. The third floor had one area for smoking which was located on a patio outside the day room. The area was observed to have ashtrays on the tables with cigarette butts in the ashtrays and on the floor of the area. The only trash container was one plastic trash can with a lid and was observed to have miscellaneous trash in the can. The second floor had two adjoining smoking areas outside the day room which were separated by a metal fence. Entrance to each area was from separate patient care areas and the fence prevented entry into the opposite patio. The smaller of the two areas had a container with an opening only large enough for placement of single cigarette butts and no other trash container. The larger of the two areas had ash trays on the table and had one plastic trash can for general trash disposal.
None of the three areas had a metal container with self closing lid into which the ashtrays or cigarette butts could be disposed of and only to be used for disposal of lit or extinguished cigarettes.
Review of the facility policy titled "smoking policy" and dated as revised in December of 2010 showed under policy that inpatients will be allowed to smoke on the designated patio with approval from their physician.
