Based on record review, document review, and interview the hospital's laboratory services failed to meet the needs of it's patients.

Findings:
1) Review of patient records and laboratory tests showed that laboratory staff used wrong patient's blood for typing and cross matching for the purpose of blood administration. This error led to the hospital administering the wrong blood type to a patient which brought about a cascade of problems outside of the hospital's ability to treat. This resulted in the patient being transferred to a higher level facility.

2) Document review showed that laboratory staff had not reviewed policies and procedures for lab or blood bank during the previous year.

3) Document review showed that blood Bank policies and procedures were not current.Procedures state they will be reviewed annually. Last review was 4/28/2015.
